Provide in-home patient assessments and support telehealth NP-led visits; document in the EMR; communicate clinical findings to the NP and care team; identify safety concerns; educate patients and reinforce care plans; support care coordination between in-home visits and virtual care; ensure HIPAA-compliant practices and adherence to clinical protocols; drive travel for patient visits (car stipend provided); collaborate with NPs and interdisciplinary team to deliver comprehensive home health services.
Required Qualifications
