Job Summary
Conduct patient visits as specified by the physician's treatment plan, making skilled observations of physical and mental conditions and documenting care accurately. Collaborate with the RN Case Manager and healthcare team members to ensure effective patient care while adhering to clinical protocols and federal, state, and county regulations. Report changes in patient condition to supervisors and the attending physician in a timely manner and provide education to patients and caregivers regarding care plans and health management. Complete and submit all required documentation accurately and promptly while demonstrating sensitivity to patient and family needs.
Required Qualifications
- Graduate of an accredited practical nursing program
- Licensed to practice in the respective state
- Current CPR or BLS certification with hands-on demonstration
- Minimum of one year of nursing experience
- Valid Driver's license
- Compliant auto liability insurance coverage as per organizational policy
Desired Qualifications
- One year of home care or hospital nursing experience
- Excellent clinical skills
- Strong written and verbal communication abilities
- Ability to work effectively within a multi-disciplinary team
- Strong organizational and time management skills
- Experience with electronic health records (EHR) systems, such as HomeCare HomeBase
