Job Summary
Provide 1:1 care for clients following their plan of care while maintaining their dignity and independence. Administer medications, feedings, and oxygen; perform ostomy care and provide assessments per the plan. Accurately document nursing actions, communicate with family, case managers, and physicians regarding medical changes, and coordinate supplies and medications to ensure adequate stock. Ensure all patient needs are met according to physician instructions. Full-time home visits Monday through Friday in Nashville, Davidson County, TN.
Required Qualifications
- LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
- Valid driver's license
- Acceptable driving record
- Proof of car insurance
- CPR professional certification
Desired Qualifications
- 1+ years of nursing experience
- Home Health care experience
