Job Summary
Provide in-home direct patient care according to the prescribed plan of care and nursing scope of practice; monitor patient conditions (vitals, wounds, infection indicators); administer medications, insulin, and IV fluids; coordinate with providers and other health professionals to achieve optimal patient outcomes; document thoroughly and communicate changes promptly; support patients and families with dignity, comfort, and independence in home health settings.
Required Qualifications
- Graduate of a practical (vocational) nursing program for LPN/LVN
- Current unrestricted LPN/LVN license in the state(s) of practice
- Reliable transportation to/from care sites
- Experience with trach and/or ventilator care preferred but not required
