Provide in-home direct patient care according to the provider's prescribed plan of care while closely monitoring progress and recovery. Assess and document patient conditions, administer medications including insulin and IV fluids, and inspect wounds or assist with personal hygiene. Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes. Current unrestricted LPN licensure and reliable transportation required; practical trach or ventilator experience preferred. Compensation ranges from $23 to $28 with flexible full-time, part-time, and PRN scheduling options.
