Job Summary
Provide in-home direct patient care according to the provider's prescribed plan of care while monitoring progress and recovery. Assess and document patient conditions including vitals, blood, stool, urine, and glucose levels, reporting changes as needed. Administer medication, insulin, and IV fluids with thorough documentation. Inspect patients for injuries, infections, and bedsores, delivering appropriate treatments and assisting with personal hygiene. Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes. Requires a current LPN license, 1200 hours of experience, and reliable transportation. Full-time days or nights shifts available with competitive pay ranging from $22 to $28.
Required Qualifications
- Graduate of a practical (vocational) nursing program for LPN/LVN
- Current license as a LPN/LVN in the state(s) in which candidate completed at least 1200 hours of experience as an LPN/LVN
- Completion of the OCan contact ffice's preceptor program
- Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position
- Reliable transportation to/from care sites and/or work locations

- Practical trach and/or ventilator experience
