Job Summary
Provide skilled nursing services to patients in their homes under the direction and supervision of a Registered Nurse. Assess, plan, implement, and evaluate patients' needs and care, offering pertinent education, treatment, and emotional support for injury recovery, illness management, and chronic disease. Demonstrate excellent assessment skills and technical proficiency while anticipating changes in clinical status. Maintain excellent rapport with patients and family members, communicating professionally with doctors and healthcare professionals regarding medication and plan of care changes. Ensure concise and accurate documentation of all patient interactions and care adjustments.
Required Qualifications
- Graduate of an accredited practical nursing program
- Current LPN license within the hiring state
- Must meet all State Board of Nursing requirements
- Must be licensed driver
- 1 year of experience
- CPR (Cardiopulmonary Resuscitation) certification
- LPN-IV Certification (must be previously completed or successful completion of course within 3 months of hire for LPNs in Missouri)
