Job Summary
Coordinate and communicate with clinical team members regarding patient condition changes and plan of care revisions. Perform comprehensive physical and psychosocial reassessments at each home visit, assessing pain and documenting outcomes. Administer medications following Visiting Nurse Association policies, ensuring rights and proper procedures are met. Schedule and prioritize nursing visits based on patient acuity while maintaining agency productivity standards. Order durable medical equipment and arrange lab results per medical orders. Maintain accurate documentation of nursing care, medication profiles, and patient/family participation in the care plan.
Required Qualifications
- Minimum graduate of an accredited LPN program
- Two (2) years of recent experience in acute care facility or homecare
- Demonstrated clinical competence in relevant area of nursing practice
- Effective and well developed communication skills
- Ability to deal well with people
- Ability to multi task
- Ability to utilize problem solving skills
- Ability to confront and resolve issues
- Ability to motivate others
- Ability to work as a member of a team
- LPN - Licensed Practical Nurse - Board of Registration in Nursing (Massachusetts)
- Must be available for 24 hours 8-430, M-F days
- Must be available for 1 Saturday every 4th week
- Must be available for 1 holiday per year
