Job Summary
LPN Home Care Grand Rapids Float role with Corewell Health West Visiting Nurses Team. Provides in-home nursing across West-Central Michigan, visiting 5-7 patients per shift under Nursing Manager supervision, using clinical judgment to assess, monitor, plan, and implement treatment plans. Works autonomously in home health, collaborating with an interdisciplinary team to deliver patient-centered care in patients’ homes. Requires an LPN license in Michigan, a valid MI driver’s license, and BLS certification within 90 days; benefits package and sign-on bonus are noted. Typical shift is 8:00 a.m. to 4:30 p.m., full-time, 40 hours per week, with travel across listed covered areas in Grand Rapids and surrounding counties.
Required Qualifications
- High School Diploma or equivalent
- MI Driver’s License Required
- MI License Practical Nursing (LPN) required
- AHA or ARC Basic Life Support (BLS) Required within 90 days
