Job Summary
LPN Home Care role in Grand Rapids, MI within Corewell Health West Visiting Nurses. The LPN provides in-home clinical nursing care under RN/Mid-level supervision, supports autonomous patient visits, and delivers care to approximately 5-7 patients per shift across West-Central Michigan communities (Ludington to Gun Lake). Responsibilities include contributing to assessment, planning, implementation, and evaluation of patient care; informing patients and families about treatments and medications; coordinating with the health-care team; and documenting care in patient records.
