LPN - Home Care - Casual

Provide care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety to patients in their home environment. Participate in ongoing interdisciplinary assessments and the development of care plans while recognizing and reporting changes in patient condition to the RN supervisor. Counsel and educate patients and families on healthcare needs and complete clinical notes addressing patient progress. This role offers flexible scheduling of patient visits within the Twin Cities Metro Area for a Licensed Practical Nurse with an active Minnesota license.
