Job Summary
LPN Home Care role with Corewell Health West Visiting Nurses Team in West-Central Michigan. Autonomous home-health nursing visits with 5-7 patients per shift, traveling to patient homes from Ludington to Gun Lake area. Under direction of RN/Mid-level Provider, contributes to assessment, planning, implementation, and evaluation of patient care; communicates treatments and medications to patients and families; documents care in patient records. Comprehensive benefits and sign-on bonus offered. Eligible candidates include experienced RNs and LPNs as well as new grads; job emphasizes independent nursing skills, critical thinking, and home-based care.
Required Qualifications
- High School Diploma or equivalent
- MI Driver’s License
- MI License Practical Nursing (LPN)
- AHA or ARC Basic Life Support (BLS) within 90 days
