Job Summary
Conduct patient assessments, administer prescribed medications, and perform nursing treatments such as urinary catheterization and sterile dressing changes for home care patients. Implement and evaluate individual plans of care while documenting visits electronically within 24-48 hours. Reconcile medications and report clinical data or symptoms indicating health declines under the direction of a Coordinator of Care RN. Requires NY State LPN license, one year of med/surg experience, a driver's license, and good computer skills.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Must have a drivers license and car
- Good computer skills
