Job Summary
Conduct advanced clinical home care services including patient assessment, medication administration, and nursing treatments such as urinary catheterization and sterile dressing changes. Perform revisits to patients based on skilled needs, implement care plans, and evaluate treatment outcomes under RN direction. Document visits electronically within 24-48 hours and report clinical data regarding patient health status. Collect and communicate signs of health decline, reconcile prescriptions, and provide patient/family education. Requires NY State LPN license, one year of med/surg experience, a driver's license, and car.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Must have a driver license and car
- Good computer skills
