LPN-Health Coordinator – Full time Monday-Friday; includes working some on call weekends

Job Summary
Provide general nursing care to CHESCO Services individuals per local, state, and federal guidelines, including routine procedures, vital signs, and documentation of care plans. Coordinate services with outside health agencies and serve as the primary liaison for community professionals. Schedule medical appointments, manage drug reviews, and ensure appropriate medication administration while monitoring discrepancies in medication records. Maintain current medical charts, provide health counseling on diet and hygiene, and participate in interdisciplinary team meetings. Perform 24-hour coverage with shared weekend and holiday on-call rotations. Requires state nursing certification, CPR/First Aid, and ability to work double shifts as necessary.
Required Qualifications
- High school diploma
- Completion of approved school of nursing
- State certification by S.C. Board of Nursing
- Certification in First Aid
- Certification in CPR
- Certification in CPI (non-crisis intervention)
- Subject to annual TB screening
- Valid driver's license from state of residence
- Ability to operate motor vehicle for 15 passengers or less
- Proof of liability insurance

- Ability to update Coordinators on medical status
- Ability to maintain and upkeep medical charts
- Ability to ensure medical charts are current
- Ability to provide and document staff/consumer/caregiver in-services/training
- Ability to provide health care counseling
- Ability to participate as member of interdisciplinary team
- Ability to prepare required nursing reports
