Job Summary
Assess patients, manage medications, and perform nursing treatments such as catheterization and sterile dressing changes under RN direction. Reconcile prescribed plans of care, document visits electronically within 24-48 hours, and report clinical data or signs of health decline. Implement and evaluate home care plans for patients recovering from illness, injury, or surgery. Requires NY State LPN license, one year of med/surg experience, a driver's license, and strong computer skills.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Must have a driver license and car
- Good computer skills
