Job Summary
Provide direct patient care under the supervision of a registered nurse in a home-health setting across Groveport, OH and surrounding areas. Implement the plan of care, perform treatments, and collaborate with the care team to achieve positive patient outcomes. Document accurately and timely in accordance with the plan of care. Educate patients and families on self-care and diagnosis-related information. Coordinate home health services with other disciplines (HHA, OT, PT, MSW, ST, Dietitian) and report identified needs to the RN or Clinic Manager. Prepare and use equipment and supplies per aseptic technique, participate in ongoing professional development, and perform additional duties as assigned, including care for hospice patients as census allows. The role requires on-road patient visits with occasional training at the local office or remote work as applicable, with an expected travel radius of ~30 miles and mileage reimbursement.
Required Qualifications
- Graduate of an accredited practical nurse or vocational nursing program
- Current license as a Practical Nurse (LPN) or Vocational Nurse (LVN) in Ohio
- Current licensure in state and CPR certification
- Must be a licensed driver with a reliable automobile
- Recent TB test
- 3 years of nursing experience, preferred
- Prior experience in community/home health and/or medical/surgical, preferred
- Comply with accepted professional standards and practices
- Demonstrates good verbal and written communication, and organization skills
- Must be able to lift, position or transfer patients and lift supplies and equipment
