Job Summary
Assess patients, manage medications, and implement care plans for homebound individuals recovering from illness, injury, or surgery. Perform revisits based on skilled needs, document visits electronically within 24-48 hours, and report clinical data regarding health status changes. Administer prescribed treatments including urinary catheterization and sterile dressing changes under RN direction. Requires NY State LPN license, one year of med/surg experience, a driver's license, and car.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Must have a driver license and car
- Good computer skills
