Job Summary
Conduct patient assessments, administer prescribed medications, and perform nursing treatments such as urinary catheterization and sterile dressing changes under RN direction. Revisit patients in their homes based on skilled needs to implement plans of care and evaluate treatment progress. Document visits electronically within 24-48 hours and report clinical data regarding health status or signs of decline. Requires NY State LPN license, one year med/surg experience, a driver's license, and car.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Must have a drivers license and car
- Good computer skills
