Job Summary
Manage after-hours nursing needs by responding promptly to phone calls and making patient visits as necessary. Receive patient assignments from the Patient Care Manager and coordinate optimal care with RN Case Managers, hospice staff, and other agencies. Cultivate communication and bereavement skills while maintaining regular contact with attending physicians and new patient families. Obtain data on physical, psychological, social, and spiritual factors to participate in the Plan of Care and maintain accurate patient records. Teach caregivers and volunteers on medication administration, personal care, mobility, and disaster management. Inform the RN Case Manager of unusual issues and provide appropriate support at time of death. Share in providing 24-hour, seven days a week coverage. Full-time role with 3-12 hour shifts; hospice experience preferred.
Required Qualifications
- LPN-graduate of an accredited LPN program
- Current state license as a Licensed Practical Nurse
- Valid driver's license
- Valid Nursing License
Desired Qualifications
- Hospice experience preferred
- Minimum of one year of experience in hospice, home health or geriatric care preferred (but not required for the right candidate)
- Must be of high integrity including maintenance of confidential information including all patient records
- Works as a team player, following directions and adhering to defined care plans
- Demonstrated commitment to the hospice philosophy of care
- Can successfully pass background checks
