Job Summary
Conduct advanced clinical home care services, including patient assessment, medication management, and patient/family education. Perform revisits to patients based on skilled need, implementing the plan of care and evaluating treatment outcomes. Document visits electronically within 24-48 hours and accurately record clinical data, reporting any signs of health decline. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes under RN direction. Requires NY State LPN license, one year med/surg experience, and good computer skills. Earn a $10,000 sign-on bonus or student loan assistance.
Required Qualifications

- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
