Job Summary
Provides direct and indirect care in the inpatient rehabilitation setting under RN supervision; administers medications and follows the five medication rights; performs head-to-toe assessments and pain assessments, revises care plans as indicated, and coordinates with physicians, the DON, and the Therapy Department to integrate nursing with rehabilitation therapies. Responsible for patient safety, wound care, IV access, documentation, and educating patients and families about care plans and discharge needs. Demonstrates professional communication with the care team and adapts care to adult and geriatric populations within the LPN scope of practice.
Required Qualifications
- Current Licensed Practical Nurse (LPN) license
- Ability to perform nursing duties under RN supervision
- knowledge of medications and administration based on patient age and condition
- follows medication rights and reduces medication errors
- ability to perform head-to-toe assessment and reassessment
- provides wound care and IV administration under scope of practice
