LPN - Full Time, first shift

Accurately assess, communicate, and document residents' status while observing and reporting significant changes in behavior and health to the Resident Care Director, physician, family, RCAs, and Executive Director per state regulations. Direct and supervise daily work assignments of Resident Care Assistants, closely mentoring Lead Resident Care Assistants in all job aspects. Coordinate care needs with community providers via an effective case management process and effectively balance service demands with supporting resident independence. Recognize and assist in the prevention of elder abuse, neglect, and exploitation, responding to emergencies and personal emergency response systems with knowledge of when to call for backup. Full-time, first shift, every other weekend required; hourly rate $34.75–$38.75.
