Job Summary
Licensed Practical Nurse (LPN) in long-term care setting. Provides professional nursing care for residents under supervision, administers medications and treatments as ordered, assesses residents, documents outcomes, and maintains confidentiality. Works as part of an interdisciplinary team to implement care plans, supports resident education, and communicates effectively with residents, families, and care providers. 40 hours per week, days 6am-2:30pm, with every other weekend and rotating holidays; includes shift differentials on weekends. Must have an accredited practical nursing program graduate credential and an unencumbered LPN license; associate degree in practical nursing preferred.
Required Qualifications
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse
- Associate degree in practical nursing is preferred
- Current unencumbered LPN license with State Nursing board and/or multi-state licensure privileges
- Ability to perform testing and administer medications/treatments as ordered
- Ability to communicate clearly and maintain confidentiality; CEU/compliance requirements
- Demonstrates knowledge of nursing principles and patient care in long-term care
- Experience meeting care plan standards and collaborating with interdisciplinary team
- Ability to participate in care plan development and resident education
