Job Summary
Licensed Practical Nurse (LPN) for IMPACT Day Services Program in Middletown, CT. Provides health, well-being, and development support for individuals with ID/DD and physical disabilities; collaborates with staff to promote inclusion and emotional growth. Must have trach care experience, CT LPN license and CT driver’s license. CPR/First Aid, PMT, and DDS Medication Administration Certification are required or obtainable. Schedule is 8:30 am–2:30 pm (30 hours/week), with flexible scheduling across programs. Non-profit employer offering competitive salary and benefits; Equal Opportunity Employer.
Required Qualifications
- Valid CT LPN License
- Valid CT Driver’s License
- Typically, 1-3 years on the job experience
- Experience working with individuals with ID/DD and physical disabilities exhibiting challenging medical, physical, and behavioral needs
- Willingness to work a flexible schedule; including different programs as needed
- Have or be able to obtain CPR/First Aid, PMT, and DDS Medication Administration Certification, as required
- Trach care experience
