LPN - Freeport NY

Conduct advanced clinical home care services including patient assessment, medication management, and patient/family education under RN direction. Perform skilled revisits to patients recovering from illness, injury, or surgery, implementing the plan of care and evaluating treatment outcomes. Accurately observe, measure, record, and report clinical data related to health status, documenting visit details electronically within 24-48 hours. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization, sterile dressing changes, and tracheal suctioning as indicated in the physician's plan. Maintain a valid NY State LPN license, one year of med/surg experience, a driver's license, and strong computer skills.
