Job Summary
Conduct advanced clinical home care services including patient assessment, medication administration, and nursing treatments such as urinary catheterization and sterile dressing changes. Perform revisits to patients based on skilled needs under the direction of a Coordinator of Care RN or RN, implementing care plans and evaluating treatment outcomes. Accurately collect, communicate, and document clinical data and signs of health decline within 24-48 hours. Requires NY State LPN license, one year of med/surg experience, a driver's license, and strong computer skills.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Must have a driver license and car
- Good computer skills
