LPN - Flexible Schedule

Job Summary
Provide in-home direct patient care according to the provider's prescribed plan of care and nursing scope of practice, closely monitoring progress and recovery. Assess and document patient conditions and report changes as needed (vital signs, glucose, etc.). Administer medications including insulin and IV fluids, document thoroughly, and perform wound care and dressing changes. Assess for further injuries or infections and deliver appropriate treatments. Collaborate with providers and other health professionals to ensure optimal patient outcomes. Requires completion of an approved practical nursing program and active LPN/LVN license, with CPR/BLS/First Aid certification and reliable transportation for care-site travel. Flexible scheduling available (Full-time, Part-time, PRN).
Required Qualifications
- Graduate of a practical (vocational) nursing program for LPN/LVN
- Current license as an LPN/LVN in the state(s) of practice
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
