LPN - Flexible Schedule

Job Summary
Provide in-home direct patient care according to the prescribed plan of care (nursing scope of practice) and closely monitor patient progress. Assess and document patient conditions, report changes as needed (vital signs, wound care, stools, urine, glucose, etc.). Administer medications, including insulin and IV fluids, and thoroughly document. Inspect wounds, perform dressings, and assist with personal hygiene. Assess for further injuries, infections, bedsores, and deliver appropriate treatments. Collaborate with providers, aides, and other health professionals to ensure excellent patient outcomes. Scheduling is truly flexible with Full-time, Part-time, and PRN options; in-home care setting with a focus on work-life balance. Requires completion of a practical nursing program and a current LPN license in the relevant state(s), plus CPR/AED/BLS/First Aid certification and reliable transportation to care sites. Required Qualifications
- Graduate of a practical (vocational) nursing program for LPN/LVN
- Current unrestricted LPN/LVN license in the state(s) where candidate completed at least 1200 hours of experience
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- At least 1200 hours of experience as an LPN/LVN
- Practical trach and/or ventilator experience preferred, not required
