Job Summary
Provide in-home direct patient care per the provider's plan of care (nursing scope of practice) with close monitoring of progress; assess and document patient conditions; administer medications, insulin, and IV fluids; inspect and care for wounds and assist with hygiene; assess for further injuries or infections and deliver appropriate treatments; collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes; requires LPN license in the state of the position and CPR/BLS/First Aid certification; must have reliable transportation to care sites; compensation in the $23-$28/hour range with sign-on bonus and flexible scheduling (FT/PT/PRN).
Required Qualifications
- LPN license
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites
- Practical trach and/or ventilator experience preferred, not required
