LPN (Fee For Service)

Conduct skilled home care revisits for patients recovering from illness, injury, or surgery under the direction of a Coordinator of Care RN or RN. Implement and evaluate individual plans of care, including patient assessment, medication reconciliation and administration, and advanced treatments such as urinary catheterization and sterile dressing changes. Accurately collect, communicate, and document clinical data and any signs of health decline within 24-48 hours. Maintain a NY State LPN license, one year of med/surg experience, a valid driver's license with a car, and strong computer skills.
