Map out and oversee patient recovery processes at home by performing revisits, implementing care plans, and evaluating treatment outcomes. Provide advanced clinical services including patient assessment, medication management, and education. Accurately observe, measure, record, and report clinical data while documenting visits electronically within 24-48 hours. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes under RN direction. Collect and communicate signs of health decline immediately. Requires NY State LPN license, one year med/surg experience, driver's license, and strong computer skills.
