Licensed Practical Nurse (LPN) to provide skilled nursing care at Evercare of Granite City. Responsibilities include direct patient care such as vital signs, wound care, and medication administration; assisting with activities of daily living; developing and implementing individualized care plans; maintaining accurate records; infection control; collaboration with an interdisciplinary team; and participation in ongoing education and regulatory training to ensure best practices.
