Job Summary
Provide nursing care in accordance with resident assessments, care plans, and facility policies while administering medications, treatments, enteral feedings, and IV therapies within the LPN scope of practice. Monitor resident conditions, promptly communicate changes to providers, and accurately document all care, treatments, observations, and medication administration. Supervise and support Nursing Assistants to ensure quality resident care, maintain up-to-date resident information during shift changes, and follow infection control and confidentiality standards. Participate in interdisciplinary care planning, quality improvement initiatives, and assist with admissions, transfers, and other assigned nursing responsibilities. Required Qualifications
- High School Diploma or equivalent
- Associate's Degree
- Valid NYS Licensed Practical Nurse (LPN) License
- Current CPR Certification
- Knowledge of medication administration
- Knowledge of infection control
- Knowledge of resident care practices
- Ability to supervise nursing assistants
- Ability to work effectively as part of an interdisciplinary team
- Compassionate, resident-centered approach to care
