Conduct advanced clinical home care services, including patient assessment, medication management, and patient/family education. Perform revisits to patients based on skilled needs, implementing care plans and evaluating treatment outcomes. Accurately observe, measure, record, and report clinical data related to patient health status, including documenting signs of decline. Administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes under RN direction. Document visits electronically within 24-48 hours. Requires NY State LPN license, one year of med/surg experience, and good computer skills.
