Job Summary
Conduct advanced clinical home care services including patient assessment, medication management, and patient/family education. Perform skilled revisits to patients recovering from illness, injury, or surgery, implementing the plan of care and evaluating treatment outcomes. Accurately observe, measure, record, and report clinical data related to health status, while documenting visit details electronically within 24-48 hours. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes under RN direction.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Good computer skills
