LPN - Eastchester NY

Conduct patient assessments, manage medications, and perform nursing treatments such as catheterization and sterile dressing changes for home care patients recovering from illness or surgery. Revisit clients based on skilled needs, document visits electronically within 24-48 hours, and report clinical data regarding health status declines. Administer prescribed medications and implement care plans under the direction of a Coordinator of Care RN or RN. Maintain accurate records of symptoms and treatments while ensuring compliance with physician protocols. Requires NY State LPN license, one year of med/surg experience, and a driver's license.
