LPN - Eastchester NY

Conduct advanced clinical home care services including patient assessment, medication management, and patient/family education under the direction of an RN. Perform revisits to patients based on skilled need, implement care plans, and evaluate treatment progress. Document visits electronically within 24-48 hours and accurately record clinical data, reporting any signs of health decline. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes. Requires NY State LPN license, one year med/surg experience, driver's license, and car.
