Job Summary
Assess patient and family unmet health and social needs, develop care plans based on mutual goals, and monitor adherence to facilitate changes. Provide effective communications to improve health literacy and ensure tracking of test results, medication management, and follow-up appointments. Facilitate access to medical providers and support specialists while coordinating communication between specialists and the Primary Care Physician post-discharge. Perform regular visits for patient and family support and education. This full-time onsite role at Northern Westchester Hospital in Mount Kisco, NY, requires a Monday through Friday schedule from 6:30 a.m. to 3 p.m. or 7 a.m. to 3:30 p.m.
Required Qualifications
- Active and unrestricted LPN license in the state of New York
- 5+ years of clinical nursing experience in home healthcare, inpatient hospital, ED/urgent care or similar clinical nursing environments
- Experience caring for chronic disease patients
- Experience with navigation of local medical and social support systems
- Ability to support onsite schedule
- Monday - Friday 6:30a - 3p or 7a - 3:30p
- Candidates are required to pass a drug test before beginning employment
Desired Qualifications
- Experience in clinical or community health settings
- Experience with Electronic Medical Records and Microsoft Excel
