Job Summary
Maintains delivery of health care for individuals with developmental disabilities in community-based homes; provides preventive, routine, and emergency healthcare; administers medications and treatments; ensures compliance with physicians' orders, agency policies, and regulatory requirements; accompanies individuals to appointments and may transport them; participates in training, on-call rotation, and shift reports; performs housekeeping and laundry as assigned; maintains documentation and daily logs; requires NYS LPN license, CPR, driver’s license, and SCIP-R training; performed duties include patient monitoring, medical administration, and coordination with RN and supervisory staff.
Required Qualifications
- Current NYS license to practice as a Licensed Practical Nurse
- Valid Driver’s License that meets agency policy
- CPR certification
- Ability to meet physical requirements including standing, walking, sitting, bending, stooping, squatting, kneeling and climbing
- Physical agility and ability to react to emergency situations with SCIP-R certification
