Job Summary
Monitor health status, administer medications and specialized treatments, and coordinate medical appointments in an Individualized Residential Alternative setting. Perform wound care, assist with activities of daily living, and prepare healthy meals with strict portion control. Maintain accurate documentation in eVero and QuickMar, train direct support professionals on health protocols, and mentor individuals through social interactions and crisis intervention strategies. Provide transportation to community sites and ensure a safe, clean environment while developing independent living skills. Maintain LPN licensure and attend required ratio-ready trainings.
Required Qualifications
- Licensed Practical Nurse
- 2-5 years of previous nursing experience with IDD individuals with co-occurring behavioral health challenges or hospital nursing and/or nursing home experience
- Strong written and verbal communication skills
- Self - Starter
- Strong organizational skills
- Excellent problem solving and time management skills
- Maintains LPN licensure
- Must be capable to sit or stand in front of a computer for long-periods of time
- Able to work in open space floor plan
- Must be capable to move throughout work day and follow people served throughout community
- Work alongside co-workers within 3 feet
- Must be able to move in tight spaces
- Occasional lifting of 25 pounds or more
Desired Qualifications
