Job Summary
LPN Day Shift in a post-acute setting (Albuquerque, NM) focused on delivering nursing care per the nursing care plan under RN direction. Responsibilities include implementing basic and rehabilitative nursing care, developing individualized care plans through team conferences, administering medications, performing IV therapy within LPN scope, documenting in EMR, coordinating communication among care team and family, providing health teaching, and contributing to plan of care and discharge planning. Requires strong communication, clinical troubleshooting, adherence to HIPAA and infection control, and the ability to work in a fast-paced, high-performance team environment.
Required Qualifications
- High School Diploma or Equivalent
- Graduate of an accredited LPN program
- Valid State Appropriate LPN license
- Current CPR certification
