Job Summary
In-home Licensed Practical Nurse (LPN) on Day Shift provides direct patient care per the plan of care, including administering medications (insulin, IV fluids), monitoring progress, wound care, hygiene assistance, and collaborative care with providers and across the care team to ensure outcomes. Flexible scheduling available (FT, PT, PRN); responsibilities include assessment, documentation of conditions (vitals, glucose, etc.), and delivering appropriate treatments at the patient's home. Requires current LPN licensure in the state of assignment and reliable transportation to care sites. Compensation: $23-$28 per hour.
Required Qualifications
- Current unrestricted license to practice as a Licensed Practical Nurse (LPN) in the state associated with this position
- Reliable transportation to/from care sites and/or work locations
- Practical trach and/or ventilator experience preferred, not required
