LPN - Day Shift

Provide in-home direct patient care according to the provider's prescribed plan of care and nursing scope of practice; closely monitor progress and recovery, assess and document patient conditions (vital signs, labs, wound/bedsores), administer medications including insulin and IV fluids with thorough documentation, inspect and care for wounds and assist with personal hygiene, assess for additional injuries or infections and implement appropriate treatments, and collaborate with providers, aides, and other health professionals to achieve optimal patient outcomes. Requires current unrestricted LPN licensure in the applicable state and reliable transportation to care sites; practical trach and/or ventilator experience preferred but not required. Compensation: $23-$28 hourly.
