LPN - Continuous Care PM shift

Job Summary
Continuous Care LPN on PM shift (8:00 PM–8:00 AM) with 1:1 patient ratio serving Sarasota-Manatee-Desoto-Sarasota counties. Under RN supervision, provides bedside clinical observations, prescribed care, and emotional support to patients and families; collaborates with RN to process physician orders to maximize patient comfort. Requires Florida LPN licensure, nursing program graduation, and at least 1 year of acute care experience; hospice LPN experience preferred. Mileage reimbursement is offered. Eligible patients fall under Empath Health’s hospice and continuous care framework.
Required Qualifications
- Graduate of an approved nursing program
- Current Florida LPN license
- Minimum one year experience in an acute care setting; hospice LPN preferred
