Job Summary
LPN Continuous Care: Provide one-to-one crisis hospice care under the ICC service for patients in home settings or in facilities. Work eight to 12 hour shifts (night), spend time with interdisciplinary team weekly, and help maintain patient comfort and dignity at the end of life. Must hold an active nursing license and have at least one year of hospital nursing experience with reliable transportation. Location: Cincinnati, OH, primarily in the patient’s home with occasional facility-based care; role emphasizes compassionate, holistic end-of-life support and collaboration with social workers, physicians, chaplains, and other nurses.
