LPN Continuous Care- Nights

Provide one-to-one crisis care for hospice patients requiring high-intensity clinical supervision in private residences or assisted living facilities. Deliver compassionate, responsive support to elevate patient quality of life and reduce caregiving burdens on families. Work eight- to 12-hour shifts, visiting the local program office weekly to collaborate with an interdisciplinary team including social workers, physicians, and chaplains. Maintain patient welfare as the central focus while extending comfort and dignity during the end-of-life journey.
