Job Summary
LPN - Continuous Care, Nights: Provide bedside clinical observations and prescribed care under RN supervision for hospice patients in a 1:1 care setting on 12-hour night shifts (8:00 PM–8:00 AM) across Manatee, Charlotte, DeSoto, and Sarasota counties. Collaborate with the RN case manager to maximize patient comfort, process physician orders, and support triage/Care Centers overnight. Requires Florida LPN license and nursing program graduation; hospice LPN experience preferred. Required Qualifications
- Graduate of an approved nursing program
- Current Florida LPN license
- Minimum one year experience in an acute care setting; hospice LPN preferred
