Job Summary
Direct client care under the supervision of the Nursing Field Supervisor/Director of Patient Services, ensuring adherence to the therapeutic plan and agency policies. Provide prescribed medications under policy, document patient status, communicate changes to RNs, participate in meetings and quality improvement activities, reinforce instructions to clients/families, and perform nursing tasks within LPN scope of practice. Requires NY LPN license, PN education, ability to communicate effectively, maintain confidentiality, and willingness to work in-home care settings with autonomous problem-solving and ability to lift 50 pounds.
Required Qualifications
- Graduate from an accredited School of Practical Nursing
- Current valid New York State license and registration as a Practical Nurse
- Minimum of one (1) year medical/surgical experience, preferred
