Job Summary
LPN position within a healthcare team in Torrington, CT. Under RN direction, perform selected nursing tasks, participate in all phases of the nursing process with data collection and reporting, contribute to care strategy in collaboration with nursing staff, and provide directed care to patients including monitoring, data gathering, and documentation. Administer medications and assist with immunizations under supervision, conduct initial data gathering for pre-visit planning, maintain charting and use of multiple EMRs (ECW, Epic), and ensure patient confidentiality in accordance with HIPAA. Maintain a safe clinical environment, support chronic care management activities, and be adaptable to move between clinic areas and satellite locations as needed. Maintains CPR certification and participates in ongoing education and competency validation.
Required Qualifications
- Current LPN license in good standing
- CPR certification
- HIPAA compliance
- Experience with EMR systems (ECW, Epic)
- Ability to perform nursing duties under RN direction
- Experience with patient data collection and documentation
- Ability to assist with immunizations and pre-visit planning
- Willingness to work in a clinic setting and potentially move to satellite locations
